Getting to a clinic is often the hardest part of staying well. Transport, waiting rooms, and short appointments wear frail patients down. We reverse that: Texas-licensed physicians and advanced practice providers come to you, stay long enough to do the work, and leave a plan everyone can follow.

Most initial visits last approximately 60–90 minutes because rushed medicine misses the story. We come prepared, and we stay until the plan is clear.

Internal Medicine & Nephrology
Dr. Salman Mallick is an experienced physician specializing in Internal Medicine and Nephrology, providing personalized medical care in the comfort of your home. With advanced training from Case Western Reserve University in Cleveland, Ohio, he has extensive experience caring for adults with high blood pressure, diabetes, kidney disease, medication needs, and other chronic conditions.
Dr. Mallick believes great care begins with listening and understanding each patient's individual needs. Through LifeCare Housecall, he takes the time to build meaningful relationships with patients and families while delivering healthcare that is convenient, attentive, and personal.

Tanveer Khan, MPAS, PA-C
Physician Assistant
Tanveer Khan, MPAS, PA-C, brings a broad clinical background and a practical, hands-on approach to patient care. His experience spans family medicine, hospital and ICU care, and clinical research, giving him a well-rounded perspective when caring for patients with both routine and complex medical needs.
A graduate of the University of North Dakota School of Medicine and Health Sciences, Tanveer is NCCPA-certified and licensed by the Texas Medical Board. He focuses on helping patients stay healthier and better manage their medical needs through ongoing assessment, education, medication management, and preventive care.
At LifeCare Housecall, Tanveer is passionate about removing barriers to healthcare by bringing dependable medical care directly to patients—helping them feel supported, informed, and comfortable in their own homes.

Samira Khan, MD
Internal Medicine
Dr. Samira Khan is a board-certified Internal Medicine physician with a strong focus on preventive care and helping patients achieve lasting improvements in their health. Her clinical experience spans adult primary care, chronic disease management, women's health, geriatric care, lifestyle health, and weight management. She completed her Internal Medicine residency training in California and Nevada and has also cared for hospitalized patients at Memorial Hermann Katy.
Dr. Khan believes healthcare should go beyond treating illness. She works with patients to identify health risks early, manage ongoing conditions, and develop realistic plans that support healthier living.
At LifeCare Housecall, her preventive approach brings comprehensive medical care into the home—particularly benefiting older adults and patients managing chronic or age-related conditions who value accessible, continuous, and personalized care.

Marketing & Client Relationship Manager
Akber Hussain brings extensive experience in strategic planning, stakeholder engagement, project management, and relationship development to LifeCare Housecall.
He leads marketing and community outreach, building strong relationships with patients, families, healthcare providers, senior living communities, and referral partners. His focus is on expanding access to LifeCare's services while ensuring a responsive and seamless experience for patients and partners.
With an approach centered on trust, communication, and lasting relationships, Akber helps strengthen LifeCare's presence and connections throughout the communities it serves.
