This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
LifeCare Housecall is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information (PHI), to provide you with this notice, and to notify you if a breach of unsecured PHI occurs.
How we may use and disclose your information
We may use and disclose PHI for:
- Treatment. To provide, coordinate, or manage your care, including with consulting clinicians, facilities, pharmacies, laboratories, and caregivers you identify.
- Payment. To bill and collect from you, Medicare, or other payers, including eligibility and coverage verification.
- Health care operations. Quality review, training, licensing, compliance, and business management related to our practice.
We may also disclose PHI when required or permitted by law, such as for public health, health oversight, judicial or administrative proceedings, law enforcement, organ donation, research under approved protocols, workers’ compensation, or to prevent a serious and imminent threat. We may contact you about appointments, care coordination, and treatment alternatives.
Other uses and disclosures not described in this notice generally require your written authorization, including most uses of PHI for marketing and any sale of PHI. You may revoke an authorization in writing except to the extent we have already relied on it.
Your rights
You have the right to:
- Inspect and obtain a copy of your PHI, with limited exceptions
- Request an amendment if you believe information is incorrect or incomplete
- Receive an accounting of certain disclosures
- Request restrictions on certain uses or disclosures (we are not required to agree in every case)
- Request confidential communications at an alternative address or phone number
- Receive a paper copy of this notice
- File a complaint without retaliation
Our duties
We will abide by the terms of this notice. We may change this notice and make the new terms effective for all PHI we maintain. The current notice will be posted on our website and is available on request.
